Targeted Case Management (TCM) Adults

Our Targeted Case Management services are designed to help adults with emotional or behavioral health challenges access the care, resources, and stability they need to live meaningful, independent lives. We provide personalized, hands-on support that helps individuals overcome barriers, strengthen coping skills, and build long-term wellness.

Who We Serve

Adults living with mental health challenges

Returning citizens reentering the community after incarceration or detention

Individuals experiencing housing instability or unemployment

Those in need of coordinated behavioral health, social, and medical services

What We Provide

Our case managers work closely with each client to ensure that care is coordinated, comprehensive, and compassionate.

Support includes:

  • Comprehensive assessment and individualized service planning

  • Linkage to behavioral health, medical, housing, and employment resources

  • Coordination of community supports and benefits

  • Advocacy to ensure access to essential services

  • Monitoring and follow-up to promote progress toward goals

  • Crisis intervention and stabilization support

  • Guidance in developing independent living and coping skills
